When I go to Audio Settings I see the following:
Output: External Mixer and Output: Card Denon DN-HC4500
Below that are the outputs and I see:
deck1 Denon DN_HC4500 chan 1 & 2
deck2 Denon DN_HC4500 chan 3 & 4
But if I click on chan 1&2, the dropdown menu that appears does not have a chan 3&4 as a selection.
Why is that?

You have to install (and use in the Audio setup) the Denon ASIO drivers to get the 3,4 channels. Please consult Denon website.http://denondj.com/products/view/dn-hc4500
EDIT.. Oh you are on Mac, scratch that... :)
So, you still need to visit Denon Website and see if there are any OSX drivers for the HC4500, cant recall.
From Applications-->Utitlities->Audio Midi Setup, select the HC4500 and click on the Speakers configuration button. Set the card to Quadrophonic mode (4 Speakers) and apply. then Open VirtualDJ and see now if the chans 3,4 are offered.
At some rare cases -not sure if the HC4500 falls into this case - a sound card with dual stereo outputs may be offered as 2 separate sound cards. So check if the drop down of the SOund card offers 2 X Denon HC4500 and choose the first option in the first line and the 2nd option in the 2nd line with both at 1,2 chans.

You will need to goto Applications-->Utilities-->Audio Midi Setup and set the Denon to 4ch / 16bit mode. By default its set to 2Ch
