Ive had this issue before off and on with different updates and it seems to be back again... The right side not only doesn't show up in the preview window, i have to actually select it for the video crossfader to of to it when mixing, i don't do this with the left deck, its just there. Hope this makes sense.

In the 4 Decks default skin -->VIDEO panel, right-click on any of the Right Video Crossfader Assign buttons to set the Right side to "auto" (all 4 buttons must be off)
